Ceftriaxone Therapeutic Alternatives

1) Whenever possible (enteral access, hemodynamically stable, patent GI tract), use oral antimicrobial therapy unless otherwise noted

2) Many antimicrobials require renal dose adjustment, consult pharmacist

3) If allergies, drug interactions or other patient specific factors preclude suggested alternatives, contact pharmacist

Amox/Clav 875mg/125mg PO BID

OR

Moxifloxacin 400mg PO q24h

OR

Moxifloxacin 400mg IV q24h

Metronidzole 500mg IV q12h

Tobramycin 5mg/kg IV q24h 

Cefotaxime 2g IV q4h 

Vancomycin (see Vancoming Dosing)

Ampicillin 2gm IV q4h
