Commonly used for gram-negative infection management

Gentamicin

Occasionally used for gram-negative infections and gram-positive synergy

Amikacin

Occcasionaly used for non-tuberculosis mycobacterial disease and drug resistant gram negative infection

Extended Interval Dosing ("Once Daily Dosing")

Multiple Daily Dosing ("Traditional Dosing")

Use Extended Interval Dosing unless:
a) CrCL < 30 ml/min
b) Pregnancy
c) Cirrhosis
d) Severe burns
If any of the above apply, use MDD
